The latest cases occurred last month at the Veterans Affairs medical center in Los Angeles. Late Thursday, the Department of Veterans Affairs confirmed that the problem had occurred and said that an investigation into the matter was continuing.

“Our deepest sympathy goes out to the veterans affected by the Avastin eye injections,” it said in a statement.

Avastin, made by Genentech, is a cancer drug but is commonly used to treat the wet form of age-related macular degeneration and other eye diseases because it costs only about $50 an injection.

That saves Medicare and patients hundreds of millions of dollars a year compared with using Lucentis, a somewhat similar Genentech drug approved for the treatment of eye diseases but costing about $2,000 an injection.

On Tuesday, the Food and Drug Administration issued an alert saying that 12 patients in the Miami area had suffered eye infections after being injected with Avastin. Some of the patients lost all of the remaining vision in their treated eye, the F.D.A. said.

Earlier this year, four patients at the Veterans Affairs hospital in Nashville also suffered infections from the bacterially contaminated Avastin. The family of one man has filed a claim for $4 million, saying the infection left the man blind and with brain damage.

In the cases in Los Angeles, no contaminant has been identified, according to medical professionals and a patient involved. The five patients, who had macular degeneration, all received their injections of Avastin on Aug. 12 at the V.A. Sepulveda Ambulatory Care Center in the San Fernando Valley.

“We all ended up in the E.R. over the course of the next few days and put together the connection,” said the patient in Los Angeles. Most of the patients lost all of their vision in the eye that received the injection, he said.

Like most news providers, AOL, which reportedly paid $30 million to acquire TechCrunch last year, prohibits reporters at its media sites, including those at The Huffington Post, from investing in the companies they cover.

But AOL has made an exception for Mr. Arrington, who has been the site’s editor. He will take a backseat role at TechCrunch, which is hiring a new managing editor. He will continue to report to Arianna Huffington, who runs AOL’s media properties.

“TechCrunch is a different property and they have different standards,” Tim Armstrong, chief executive of AOL, said in an interview.

But in the case of the proposed second bailout for Greece — the one that is supposed to make private investors feel the financial pain along with taxpayers — the biggest banks in Europe are on the road now promoting the plan.

It’s not that the banks are suddenly masochists. It’s that this first major bond restructuring in Europe’s long-festering debt crisis is shaping up as a much better deal for the banks than for the Greeks it is supposed to be helping.

Holders of the Greek bonds would get much better value than they could in the open market, while Greece would still owe a lot of money. What’s more, Greece would be surrendering a lot of its negotiating clout if, in the future, it needed to go back to the bailout bargaining table.

This week, bankers representing the Greek government — Deutsche Bank, BNP Paribas and HSBC — have been explaining to investors why it is in their interest to trade in their decimated Greek bonds, take a 21 percent loss and accept a new package of longer-dated securities with AAA backing. Those bondholders include big European banks, smaller fund managers and insurance companies.

The bond exchange is a crucial component of the more than 200 billion euro ($286 billion) in rescue packages that Europe and the International Monetary Fund have put together to support the near-bankrupt Greek economy through 2014. The German chancellor, Angela Merkel, and others insisted that banks make such a contribution to give them some political cover at home.

The part of the rescue announced in July is subject to the approval of Germany and the governments of the 16 other member nations of the euro union in coming weeks. If investors balk at the 21 percent write-down that is the price for getting a deal done, the whole package could collapse. European governments would be hard-pressed to come up with those extra funds themselves.

But with the price of Greek debt trading in some cases at 50 cents on the dollar — even lower than when the bailout deal was announced in July — the 21 percent haircut seems to be quite a bargain.

As a bonus, the new bonds would be governed by international law, rather than Greek law. That is a significant alteration of lending terms that would strengthen the negotiating hand of the bondholders if Greece eventually concluded it had no alternative but to default — even after this latest bailout.

The International Institute for Finance, the advocacy group for global banks that is also the chief architect of the deal, says that 60 to 70 percent of the financial institutions holding Greek bonds have agreed to the swap so far. That comes close to the 90 percent threshold that the Greek government has stipulated, although it is too early to predict the final outcome because Greece will not formally make the swap offer until October.

“This is an attractive offer,” said Hung Tran, a senior executive at the institute. “We are making the case that if this deal is implemented it will restore stability to Greece.”

The question remains, however, whether the banks that financed the country’s debt by buying its bonds would get off too easy — and whether the Greek government should have pushed for a larger write-down to ease its debt load.

Analysts also note Greece’s diminished bargaining power in any future debt negotiations with its bankers.

In past debt negotiations involving countries like Argentina, Uruguay and Russia, the bulk of the debt was governed by either United States or British law. That gave the biggest bondholders the upper hand in negotiating terms; they could either hold out for a better deal or challenge the governments in foreign courts.

In the case of Greece’s debt, more than 90 percent of it was issued and is governed under Greek law, as a holdover of the era preceding Greece’s entry into the European monetary union in 2001. That, legal experts say, currently gives the Athens government the flexibility, if it so chooses, to alter bond contracts and secure a more beneficial restructuring deal over the objections of its foreign creditors.

For this fiscal year, which ends Sept. 30, the budget deficit will be just over $1.3 trillion, both budget offices have projected.

That is down $329 billion, or 20 percent, from the administration’s deficit estimate at the beginning of the year, reflecting higher-than-expected revenue and lower-than expected spending. A $1.3 trillion deficit is equal to 8.8 percent of the economy, as measured by the gross domestic product, which is far above the 3 percent level that economists generally consider the maximum level desirable.

But the budget office report, released by Jacob J. Lew, Mr. Obama’s budget director, argued that the combination of savings mandated by the August deficit reduction deal and reduced spending over time in Iraq and Afghanistan, together with the expiration of the Bush-era tax cuts for high incomes, would bring the deficit to 2.2 percent of G.D.P. after a decade.

Letting the individual tax rate cuts expire for annual incomes above $250,000 as scheduled after 2012 would save more than $1 trillion through 2021, the Office of Management and Budget said — $866 billion in revenue and $166 billion in interest savings from a lower federal debt.

The importance of also realizing the separate savings called for in the budget deal is why Mr. Obama plans next Thursday to recommend “an ambitious, comprehensive and balanced deficit reduction plan,” the report said. The administration hopes to influence the special Congressional committee that was created by the deal to reach a bipartisan plan by Nov. 23, for House and Senate votes by late December.

As the Congressional Budget Office earlier emphasized, even though annual deficits are expected to decline through the decade as the economy recovers, after 2021 they will climb again because of an aging population and high health care costs that are driving up federal spending, especially for Medicare and Medicaid.

He boasted that his country would produce the fuel in much larger quantities than it needs for a small research reactor in Tehran that produces medical isotopes.

The fact that Iran is declaring that its production will exceed its needs has reinforced the suspicions of many American and European intelligence officials that Iran plans to use the fuel to build weapons or to train Iranian scientists in how to produce bomb-grade fuel.

Describing the new facilities in the interview with the Iranian news service, Mr. Abassi, who narrowly survived an assassination attempt last year, said that a 2009 proposal for the West to provide Iran with new fuel for the small research reactor, in return for an end to Iranian production of the fuel, is dead. “We will no longer negotiate a fuel swap and a halt to our production of fuel,” he told the Islamic Republic News Agency on Monday. “The United States is not a safe country with which we can negotiate a fuel swap or any other issue.”

The officials involved in the discussions about Iran said that the Bush White House asked the Central Intelligence Agency in the summer of 2008 to assess the feasibility of covert action to blow up or disable crucial elements of Iran’s nuclear facilities. But when the agency delivered the plans, they were quickly rejected, all the officials said, for fear that any kind of obvious attack on the facilities could touch off another conflict in the Middle East just as a new president was assuming office.

The options were developed in part to assess whether a physical attack on the facilities would be significantly more effective than more subtle — and deniable — sabotage of the Iranian facilities, including cyberattacks. That presentation and subsequent discussions led to a detailed exploration of Iran’s vulnerability to a sophisticated cyberattack.

“There were a range of options from the highly kinetic to the other end of the scale,” said one former official involved in the decision-making, using the military’s jargon for the use of physical force against a target. The New York Times reported in January that Stuxnet was primarily the work of the American and Israeli governments, and was the most successful example yet of a computer-based attack to destroy another nation’s physical infrastructure. But neither the United States nor Israel has ever publicly discussed how the sophisticated computer worm came into existence, or who was responsible for releasing it.

It is not clear that Stuxnet had been written or tested by the time the Bush administration explored the computer-based options in late 2008. It hit Iran roughly one year into the Obama presidency. None would speak on the record about an issue of such sensitivity, and they declined to discussed classified operations that the Bush or Obama administration approved.

Their revelation that the C.I.A. was asked to provide a range of options so late in Mr. Bush’s presidency is an indication of how worried they were about leaving office with Iran’s program still under way and relatively unimpeded.
